Application Process

STEP 1: FILL OUT APPLICATION

Fill out all fields in the following form to the best of your knowledge. Please note, this information will be shared with out membership committee.

STEP 2: PAY APPLICATION FEE

After finishing this form, continue to make your $400 application fee payment. This fee will be credited towards your first full membership payment if you’re approved. If your application is denied for any reason you will be refunded the full $400.

STEP 3: AWAIT APPROVAL

Our membership committee will review your application. If you’re approved, you’ll receive an email from us with instructions to finalize your membership. If you have any questions, please contact membership@apdahealth.org.
